Telemedicine What to Expect
BEFORE A TELEMEDICINE APPOINTMENT

Consent for electronic communication must be completed

This will be sent to you via the Telus Connect Patient Portal

You will have to create a login to complete this.

Please have your pharmacy fax number and closest lab fax number on hand enter into the form and alsoo at the time of your appointment
PRIOR TO YOUR APPOINTMENT

Prior to the appointment the patient intake form needs to be filled out through the Telus Connect Patient Portal

Please forward any headache diaries Dr. Madhani has asked you to complete prior to your appointment. Failure to do this may result in your appointment being cancelled.

You will get instructions for a virtual visits either by email or text message.
ON THE DAY OF YOUR APPOINTMENT

On the day of the appointment 15 minutes prior to your appointment time you will need to click on to the link provided to you. This will launch the telemedicine encounter and you will be placed in the waiting room in line.

Much like a regular waiting room you will have to wait for Dr. Madhani.

When it is your turn Dr. Madhani will start the conference.

Your appointment may start earlier than at or later than your stated time depending on Dr. Madhani’s availability. Appointment windows for virtual visits are (1-2 hours). Please be prepared for this in your schedule.

Dr. Madhani will make every effort to start on time, however, medicine is human endeavor and sometimes there are delays that are not in Dr. Madhani’s control.

Have your pharmacy fax number and closest lab fax number on hand to provide to the doctor.

Have your personal health care and picture ID available for verification.
AFTER YOUR APPOINTMENT

After your appointment prescriptions will be faxed to the pharmacy and requisitions will be scanned and emailed via the Telus Connect Patient Portal to you or faxed directly to the lab.
