HOW TO REFER:

Send referral forms by fax 604-428-7860. Please ensure up to date patient information is included in the referral. Please ensure patient's email address is included with the referral.  If the patient is non-English speaking please include the contact number of someone who speaks English and can communicate with the patient.

After a patient is attempted to be reached 2-3 times without success the referral will be returned with a request for new contact information. If we do not receive new contact information within 2 weeks of this request the referral will be cancelled.

If your patient has been seen by another neurologist while on the waitlist the courtesy of a cancellation of the referral would be greatly appreciated. This prevents duplicate visits which cost the healthcare system an unnecessary burden. 

​

REFERRAL REQUIREMENTS:

Referrals will not be accepted for patient who have not been assessed in person by the referring physician. 

 

The Telemedicine practice standard and Referral-Consultation Process professional guideline have been revised to clarify that patients who are referred to specialists via telemedicine must be adequately investigated and treated before the referral is sent.
