PATIENT INFORMATION
WELCOME NEW PATIENTS
You have been referred by a specialist, primary care physician or an emergency physician for a specific neurological condition or symptoms. Our goal is to provide excellent care delivered in a seamless fashion.​

Outlined below is what you can expect as a new or returning patient. ​​

PREPARING FOR YOUR FIRST VISIT
Your first appointment may be either an initial consultation or may be directly booked as and EMG/Nerve Conduction Study if deemed appropriate by the physician.

INITIAL CONSULT

If you first appointment is a consult only. The initial consult will take between 45-75 minutes. Please plan for parking accordingly including waiting time.

FOR EMG/NERVE CONDUCTION TESTING

At the time of booking you will be advised if you will have an EMG/Nerve Conduction Testing (NCS) testing.

Please wear loose clothing on the day of your testing.

If you are on blood thinners, oral anticoagulant therapy or heparin you must inform Dr. Madhani prior to the test.

Ensure that your skin is clean and that no lotions or perfumes are worn after shower/bath prior to the test. It is best to shower on the day of the test as natural oils in the skin can interfere with results and require stronger nerve stimulation.

Nerve conduction is performed on all patients after which the need for an EMG is determined by the physician on a case-by-case basis.

The EMG/NCS testing takes 60-90 minutes.

Please plan for parking accordingly including waiting time as once the assessment begins you will not be able to top up your parking.

GENERAL INFORMATION

A patient intake form will need to be completed prior to seeing the physician.

Your are asked to arrive 10-15 minutes prior to your appointment time.

You may have more than one neurological complaint, however, the majority of the clinical encounter will focus on the concern outlined by the referring doctor.

Note if you are delayed the physician will make every effort to see you, however, in the event that this is not possible you may have to rebook and be subject to a late fee to depending on the circumstance.

Respecting your time is of the utmost importance. From time to time, emergencies and unavoidable situations may arise which unfortunately may necessitate you waiting longer for your appointment than anticipated. In the event of this we thank you for your understanding and patience.

THE DAY OF YOUR FIRST APPOINTMENT

Patients for Nerve Conduction Studies should wear loose clothing on the day of the appointment.​

Before entering the building wash your hands with hand sanitizer.

Prior to entry if possible wear a mask from home

You will be provided a medical grade mask upon arrival please step outside to change your mask.

Waiting areas are strictly for patients and essential accompanying members for translation or medical history

Please have your personal health card and picture id with you

The doctor will be wearing a mask and eye protection during your visit.

After the assessment the doctor may virtually include requested family members and friends by phone to discuss the impression and care plan. If these members have accompanied the patient they should wait outside the building and if possible in the car.

WHAT TO EXPECT AFTER YOUR FIRST VISIT
A single visit may not resolve your medical issue; Dr. Madhani will inform you at the end of the visit if a follow-up visit is required. If this is the case you will need to book your follow-up appointment at your convenience. Booking of all follow-up appointments in the patient's own responsibility.

Follow-up appointments are typically shorter than the initial consult depending on the medical condition. Dr. Madhani will address the reason for referral and will not be able to address other unrelated neurological concerns. If you wish to be seen by Dr. Madhani for another unrelated concern a new referral by your GP will be required; This is to ensure that an adequate in-depth assessment is possible and that you receive the best care possible.

RESULTS

You may not be called to review all of your results.

The office will only call you to arrange a follow-up if the doctor wishes to review results of tests ordered. The need for this is assessed on a case-by-case basis.

Front office staff/medical office assistants MOA will not be able to give results over the phone.

Please be reminded that your family physician is also able to give you test results.
